Oh, this is not an easy answer.  I found a great resource in the Access
Technology Institute (ATI) "Emersion into MSWord".
Basically, you need to go to the menus under view then toolbars and make
sure the "forms area is checked, if not press enter to check it..
Then go to the MSWord doc to where to what to place an edit box and press
then release  the alt key, then press crtl + shift+tab This will bring up
the dialog box to create forms.  Press enter on the edit box and that will
place the edit box in your document.  Then get your cursor into the edit box
and press applications and properties.  Here you can change the edit box,
for example have JAWS read the name of the box as a label.  To Change this,
press alt+TO in the properties box and hit tab 3 times, this will put you
into an edit box for you to type a message for JAWS to read an identifying
label.  This took me a long time to figure out.
After creating many edit boxes, with labels, bring up the forms dialog box
by pressing and releasing ALT, then press CRTL+SHIFT+TAB.  Then tab over to
"protect" and hit enter.  This allows you to keep the text protected in the
form and tab to/from the edit boxes in your  form.  If you did the
identifying labels correctly, it's really neat to able to tab through and
hear the identity of each edit box.  Just start typing in the box and it
will replace the text in the box.
Yes, it sounds like a lot, but you can quickly create them with some
experimentation.
