Hi simon,
Your only choice with PDF documents that have been scanned is to
use an OCR program either a "blind reading system" like OpenBook
and Kursweil, or a much less expensive commercial OCR such as
Omnipage.

Of course the blind reading systems are self voicing, thus
somewhat easier to access, but cost about a thousand bucks.
Omnipage can be had for under a hundred bucks but is somewhat
more complex to set up and use.

Using OCR systems you can directly input the PDF documents into
the OCR program and then bring it into word or anyother
application of your choice.

With PDF, there are no guarantees of actual readability.  You can
usually get the text out of the document, but formatting can be
still very complex and require dealing with changing numbers of
collumns, sidebars, and all the mess sighted people seem to need
to make it comfortable for them to publish.
