Nomination forms for the 2004 Dane County Dr. Martin Luther King, Jr.
Recognition Award are now available on-line at:
www.co.dane.wi.us. Just click on "2004 Dr. Martin Luther King, Jr. Award" on
the home page. Forms are also available in the Dane County Executive's
Office, Room 421, City-County Building, 210 Martin Luther King, Jr. Blvd.,
Madison, Monday through Friday from 7:45 a.m. to 4:30 p.m. If you would like
to have the form faxed to you or if you have any questions, please call
Isadore Knox, Director Dane County Office of Equal Opportunity at 266-4192.

Completed nomination forms must submitted to the County Executive's Office
no later than 4:30 p.m. on Monday, January 5, 2004.
