The answer to your question requires discussing multiple processes. Here is a URL on how to make an accessible PDF file. http://www.webaim.org/techniques/acrobat/ This site will give you the reasons why making an accessible PDF file is difficult.
Depending on the type of PDF that you are trying to convert, you may have to use OCR to convert the image into text, you may be able to use a software package to convert the PDF to text (this is not the OCR technique), Omnipage, Kurzweil 1000 and 3000, Adobe converter, http://www.premier-programming.com/ PDF Magic or you might go through the trouble of converting the PDF into an accessible PDF document as described on the Webaim web page.

How do you take a plain PDF from adobe and give it text so that the
screen readers can recognize the text on the page? Do you have to
compile the PDF some how or is this a process that needs to be done by
hand. Also, does the process happen either from the adobe software or
from third party software?
