Judy,

My blindness was diagnosed by two different neuro-othomologists at two
different major medical centers in 1985.  At that time, one wrote a "to whom
it may concern" letter detailing my eye condition, my visual acuity, and the
fact that it is incurable.  I use this letter whenever I apply for services
or documentation of disability is needed.  The letter really says it all and
no one has asked for anything additional.  The only problem came recently
when a local transit agency bureaucrat discarded the letter and demanded
documentation within the past year.  When I questioned this I was told to go
to my primary care physician and obtain a letter.  When I explained that my
condition was permanent and incurable, which is stated in the letter,  and
that he was not qualified to make a diagnosis and a new diagnosis which
would re-duplicate the tests of 20 years ago would cost thousands of
dollars.  I asked them if they would pay for the new diagnosis.  They
refused but the woman I spoke to asked me to fax the letter so it could be
reviewed again.  I did so and it was accepted.

Some run to blindness agencies, social workers, or rehabilitation
counselors.  My endlessly photocopied doctor's letter has worked for me and
I recommend this approach for efficiently and effectively documenting
disability to obtain benefits or receive disability or blindness related
services.

BTW:  I think you are sending e-mail in html.  the tags appear in your
message, making them really messy and chatty for screen readers.
