For Windows, there's also the texthelp screenreader - demo available from
www.texthelp.com. costs UKP 30 I think. Also KeyRead from R J Cooper,
which is i think USD 99 and demo at www.rjcooper.com

People usually like to make as much use of residual eyesight as they can.
Maybe something like the BigShot Magnifier from www.aisquared.com could
be useful, too.

Unfortunately basic screenreaders can be very limited in functionality
especially when it comes to making sense of web pages.
