Please excuse this repeat, but there were so many typo's in my first
response(which looks bad for a professor), I had to correct them

Q1: What is your Name and Age?
A1: Bobby Greer, age 63

Q2: What is your Email Address?

Q3: What type of Cerebral Palsy do you have?
A3: mild spastic quariplegia

Q4: Please describe early therapies, if any? How many years?
A4: Physical and occupational therapies in the 6th, 7th and 8th grade

Q5: Please describe any adaptations made during your early school years to
accommodate your situation
A5: None

Q6: If you went on to university, please describe that experience with
regard to effects/ changes to mobility?
A6: used an electric typewriter for written assignments and sometimes on
selected exams.Used a dictaphone on  comprehensives in grad school.

Q7: Please describe your work life and any impact it had or has on your
health status with regard to cerebral palsy?
A7: taught in a university so I have encountered few limitations

Q8: Please indicate any medications you take to manage implications of
cerebral palsy, and how long taken
A8: diazepam(Valium) for 20+ years.

Q9: Have you explored alternative therapies/treatments to manage
implications of aging with cerebral palsy?
A9: none

Q10: What concerns you the most as you age with cerebral palsy?
A10: My increasing physical fatigue restricts how active I am. My lazy
lifestyle compounds this problems
