The authors state that *any* sudden dietary change can cause
physiological problems, such as a tendency to form clots. For
this reason, they urge gradual reduction of carbs, especially for
people over 45 or with known cardiovascular issues. Since their
program involves cutting carbs to 72g/day, there is probably only
a very low level of ketosis; it probably wouldn't show up on
ketostix. Since their recommendations are based on decades of
clinical observation, I wouldn't dismiss them entirely.
