Have you ever tried velcro?  You could have someone sew the button on the
loop side (to look like it was still there and then also sew patches of
velcro on shirt so that the velcro becomes the fastener.  Many of the
adapted clothing styles do this.  For people with real difficulty buttoning
up a button up shirt you could do the whole face of the shirt then you just
have to line up the two sides.
Of course, I say this knowing that I am one of the few OT's who do not sew
but, I know where to go for help at least!<grin>

problem is buttoning the top button. a tie that you actually tie can be used
to pull the collar together enough to fake out the top button being
buttoned. i know, i used that trick when i had to wear ties.
