The problem is win95 really doesn't support USB very well.
If you have your win95 cd browse to the "other" / "usb" folder,
 in it is a exe file named "usbsupp.exe"
if you run that patch it updates your usb drivers, and the yellow
mark will go away. The patch runs scandisk before it patches
so be aware that this is normal.
  I serious doubt this usb problem is related to your blue screen .vxd
errors
If you don't have this file, I'm sure a web search would find a
download site
easily.
