Dear DHFS Colleagues:
The 2004-05 Minority Health Mini-Grant Request for Applications (RFA)
are now available for the 3/1/04-6/30/04 and 7/1/04-6/30/05 funding
periods. Please share the attached RFA with your program staff and
community partners who interact significantly with minority
community-based organizations and tribes. The RFA is also available on
the DHFS web page at www.dhfs.wisconsin.gov/rfp/. Awards up to $5,000
will be granted for mini-projects or major project planning grants
through 6/30/04. A few select projects will be eligible for continuation
awards ranging from at least $15,000 to $30,000 through 6/30/05. The
application deadline is Wednesday February 18, 2004.
The grants are intended to build the capacity of local communities to
provide culturally and linguistically appropriate health resources and
services for African Americans, American Indians, Southeast Asians,
and/or Hispanics/Latinos and to eliminate racial/ethnic disparities in
health and healthcare. Applications are targeted to racial/ethnic
community-based organizations and tribes in Wisconsin.
Projects for the 2004-05 grant program should address one of the
following 4 priority areas:
(1) Community-based strategies to reduce infant mortality and low
birthweight births;
(2) Community-driven, neighborhood-based primary prevention and health
promotion models and services;
(3) Mentoring, development, and support of minority health
professionals and minority students interested in health careers; or
(4) Programs to reduce overweight and obesity among racial and ethnic
minority groups.
