And like many of these products, it can do both speech and magnification at once. The least expensive commercial product that I would consider to be in the screen reader class is Connect OutLoud from FreedomScientific.com, albeit limited to Outlook, Internet Explorer, and a handful of Windows programs. It sells for around $250.00 in the US. There was another very inexpensive screen reader called Simply Talker, but I haven't heard about it for a couple of years now.
