First of all, I'm sorry to hear your pain has got so much worse. :-( Secondly, I had horrible, sharp muscle spasms a few years ago, so I can relate to your situation. However the MRI my neuro ordered for me showed the bone spur in my neck was making my already-spastic muscles even more spastic, and you know the rest. The surgery helped a great deal, and I don't have the painful spasms, except when I've pulled something, and then WHAM! It's painsville all over again. I'm assuming you've had the MRI, the works and nothing extraordinary has shown up. Have you sustained any injuries, perhaps pulled something in a fall? Again, I'm assuming that has been ruled out. So it now becomes an issue of pain management. Is your doc a pain specialist? I know there are a lot of pain clinics who can help you at this stage of the game. And I'm guessing Bobby's got a point - that one of your meds might be enhancing the spasms so they're worse. I hope you can utilise a pump to deliver pain meds. That'd seem the best way to go at this point, at least, to me. You might want to get a second opinion. Don't give up! You might be able to go out again and fish! In the meantime, keep us posted on what happens.
