The stone for your vault may have been laying around the stoneyard since God knows when, maybe from different quarries, or maybe that was just a bad piece from the same quarry. It's also possible something's uniquely wrong where the now-bad piece was set, but that somehow seems unlikely to me.
I would say the short (and true) answer is, "who knows?," but what you can do is fix it.
