Hi,

Does any one know of a Form Program that will do the perform any or all of
the following functions:

1) Scan in a form.

2) Allow fields to be selected for data input.

3) Allow the resulting form to be saved as or placed on a web page.

4) Allow any user to be able to fill out the selected fields on a Web page
and print the completed form.

5) Or allow the resulting form to be saved as a Word file.

6) Or allow any user to be able to fill our the selected fields when opened
as a Word document and print the completed form.
