I don't have the answer, but can only add to the question.
When I use MSIE 4.01sp1 (patched+), it taps into my
Office 97Pro Word spell checker.
When looking at e-mail headers and complex messages
with lots of "quotes and replies", My replies are
often not spell checked properly.  It seems in my
case, there is something in a normal e-mail (etc.)
that blocks the spellchecker from functioning correctly.
I often have to do multiple spell checks on the same
message by moving the cursor around to different
locations inside the message.  I have considered this
a "bug" (and have my own "work-around"...)
