George,

There might be a plug-in program for Word or Wordperfect that will do this,
but the best solution would be to get a stand alone form filling program.
One that you can create a form then type into it and print it out. One such
program is PDC Form Maker:

http://www.smartform.com/  I haven't used either of these programs so don't
know how good they are.
