Hi,

maybe, you should try the latest MS Publisher - dont know the version
number -  I was very satisfied even with ver.2.0, but get the last one.

Or,there was a very good prog FORMTOOL, as much as I remember
the last version I tried was ver 2.0 - very good one w/merging and with
a lot of tamplates...
