  I use Pagis Pro - a scanning program that came with my Xerox Work Center (combination color copier/scanner/desk jet printer -- a bargain at $189).  When you scan the document you can save it as a form and then fill in the blanks, or you can save it to most other programs - such as WORD.  I highly recommend this program due to its great capabilities.
