The free version of the excellent WinPatrol will give you that option every
time spyware or something like Realplayer or Quicktime tries to insinuate
itself into your startups.

Amongst its other virtues, Winpatrol has the very useful "Delete on
reboot" feature (Read its excellent Help file which is actually more in the
nature of a security tutorial)----though I'm not sure whether that's
