Is you customer using Diamond drivers. I have seen their InControl tools do
that in several versions of video drivers.

        A customer called me this morning, saying that every time he
clicks on the shortcut to MS Dos, his computer goes to a black
screen, and then runs scandisk because it was improperly shut
down. The same thing happens when he is trying to run a DOS game.
