  I have seen three scenarios where "click and drag" would not work:

1.  I wore out the microswitch under the left button on a cheap mouse.  Note
that it would rarely see clicks or double-clicks, too.

2.  Occasionally, a busy PC might miss seeing a "key release" signal, so it
thinks the Alt, Ctrl or Shift key is still pressed.  [Note that for purposes
of these signals, left and right count as separate keys....]  This tends to
have other odd effects, too.  Hitting each of these six keys generates a
fresh "press"/"release" sequence, and when the one that was causing the
problem is released, system behaviour returns to normal.

3.  Since installing a TV card, it seems to be easy for me to get to a state
where "click and drag" no longer works -- and additional programs will not
launch.  This *appears* to relate to system resources getting too low,
although releasing resources by closing applications doesn't always fix it.
[Sometimes Norton Utilities "Anti-Freeze" seems to help.]
