If your anti-virus program isn't capable of scanning attachments when you
download your email (or if you, as do I for performance reasons, have
configured your anti-virus program not to auto-load and to run manually
solely at your discretion), then I'd recommend saving the questionable file
to your desktop and then scanning it with well-regarded and up-to-date
anti-virus software.

I use both Network Associates' McAfee 5.21 and Grisoft's AVG 6.0, but prefer
AVG hands-down for its' speed and ease-of-use.

* That said, if you're unsure of how to handle possibly infected files, it's
always best to simply delete the suspect email.*  In my experience,
unsolicited attachments from unknown sources are almost always infected with
some bug, virus or trojan: a knowledgable user can spot them 'a mile away'.
