I believe you delete the file Windows/Shelliconcache, reboot and let
Windows recreate the file on startup.

But I wouldn't swear to it, I've never tried it. Make a copy of the file
first, just in case.

TweakUI  has an icon rebuild button under "repair".  You might try that.  I
suspect it does the same thing.

Otherwise, a long shot, you could try a registry restoration--of course
you'll have to go back to a time before this problem first appeared.
