Loud continuous "rattling" is indicative of bad bearings on a case fan. If
you are seeing image anomalies like jittering, I would say that the heat is
getting to the graphics processor. If you have a video card instead of
onboard video, that could explain both symptoms and be easily fixed with a
new video card. Take a look under the hood and see what fan is making noise,
and that should give you a fair idea what to do.
