Rich,

Based on the fact that the card is recognized upon rebooting when hard
drives, etc. have all powered up, I would check your power supply next.  I
do not know about the Voodoo cards but my experience with the first GeForce
cards was that they would behave this way when the power supply was weak or
to small.  Not knowing the configuration of your computer, hard drives, etc.
and the size and age of the power supply this is just a guess.
