I have a catalogue from Cyberguys! (www.cyberguys.com) and they feature a
product called a Multimedia Extender (Item # 202 0700) that routes line,
mic, and speaker jacks, plus volume control to the front of your case.  No
SPIDIF, though.  This seems fairly close to what you are after.  Just type
"multimedia extender" into the quick search box on their site.
