Anybody else feel free to pipe in here, but it sounds to me like registry
corruption.  A refresh install of Windows will not necessarily take care of
this problem (your registry is rewritten, but sometimes corrupted areas get
rewritten as well).  I would try removing the card, making sure that the
device is removed from the Device Manager (remove it if it shows up there),
rebooting, running RegClean or some other type of registry cleaning program
(I hear Norton's is good, but have never used it myself), reboot again, then
shut down and try reinstalling the card one more time.  If this does not do
it, you may have to resort to a complete reformat and install of Windows.
That would be a last resort though.  Good luck!
