Check out DPT's PM2554UWF SmartRAID V Century Intelligent PCI-to-Fibre
Channel Controller <http://www.dpt.com/2554uwf.htm>.
There is support for Windows NT and Unixes, but not Win95/98.

Adaptec has the AHA-F940/AHA-F950 PCI-to-Fibre Channel adapters:

which do not offer RAID. They support Windows NT and Unixes.

But Adaptec does have it's AEC 7000 Series: External Canister-based
Fibre Channel-to-Ultra SCSI RAID Controllers:

OS independent. It supports all platforms with no special software
or drivers required.
