I have found that (at least with my own equipment) when I put a scanner on a powered
4 port USB hub, that the scanner HAS to be re-installed on EACH port of that hub.
Four port hub, four installs of the scanner -- one while plugged into each port...
(Either that, or keep track of the EXACT port used during the install.)

Over kill? Seems "dumb"???, but it has worked for me...

FWIW: I have heard the some things do not "like" PCI cards using USB either.......

FWIW more: When I say "powered" hub above - I have one USB2 64M flash
drive that will NOT work in a plain un-powered hub because it draws too much
power... (I guess I should try that again.... Seems dumb, even to me...)
