I had the exact same problem a couple months back...same conditions..no dial
tone, but the line worked from the wall and windows said no problems with the
modem.
To test it, run a line from your wall jack into the Line input on the modem.
Then hook a telephone up to the Phone input on the modem. Do you get a dial
tone? If not, modem is shot...that is what was wrong with mine. The connector
in the jacks on the modem were shot. I replaced it with a 30 dollar modem and
was back in business.
