Another thing that can cause the problem is a defective internal modem.
Whenever I install/reinstall an operating system I pull all cards except
for the video out and install the OS.  When completely installed turn
the machine off and install a card.  Turn the machine on and complete
the installation of the device.  Cut the machine off, install next
card....etc.
