Susan,
have you tried hydrogen peroxide?  use a cotton ball dipped in the
peroxide, and swab the ear without too far inside.  It will bubble and
your kitten will be upset, but you can wrap the kitten in a towel with
its paws tucked in so it wont scratch or bite you.  Then after you do
that, you can move into a q-tip dipped in the same stuff and go down as
far as you can without actually entering deep down into the ear.  Keep
this up for about 4 weeks, using the peroxide every other day or so.
