To date the best book I've read about  restricted carb dieting is THE
SCHWARZBEIN PRINCIPLE. Dr. Diana Schwarzbein is a practicing physician in
Santa Barbara, CA. She's an endocrinologist by training. I found her
explanations of insulin resistance and other diet-related problems to be
very thorough.
