That is prior to adaptation to ketones, which takes a day or so.
After that, the brain uses only about half as much (some
estimates say even less).  500 kcal means about 125g of glucose.
When you eat less than that, *two* processes start, one is
gluconeogenesis; the other is production of ketones.

What if the problem is not the absence of TPP but some other
factor that interferes with its proper utilization?
