Prescription meds kill because of two main reasons:

The patients don't follow the doctor's instructions and because they
don't tell their doctors about every med they're taking, even herbs and
OTC ones.

Yes, doctors and pharmacists do make mistakes and yes, they do account
for a percentage of deaths by prescription drugs but it's two-way
street. Doctors and patients need to communicate.

I question everything. When my doctor writes a script, I ask her the
directions (to make sure I understand them and she has an opportunity to
correct any errors). When I go to the pharmacist, I ask again and also
if there are any potential interactions. I also read the pamphlet that
comes with the med just to make sure there's nothing they haven't told me.

I've always asked questions. I think it's something everyone should do.
If it annoys your doctor or the pharmacist, find another one.
