We use a 3/8" diameter hollow core bit made for extracting screws that 
have had their heads broken off. You can see it here (used for another 
purpose):

Also, all of the paint specialists I have worked with use a Dremel tool 
with a 1/16" diameter x 3/8" long burr, to cut out a little cube of wood

any size needed. (it burns the wood, making smoke, and in my opinion too

much of a fire hazard)
