  16k?  Not a colour depth I've ever heard of.  But if you mean 16 BITS
(64k colours), then that and 1024x768@16 each require 512KB of video RAM.

  The simplest and most likely explanation is that the old (working) drivers
for the V550 got lost in the re-install; you may need to download these fresh
from Diamond's web site.
  However, it may be necessary to consider the possibility that the RAM on
the card has gotten damaged, and that the driver is working properly, but
finding only 512KB of usable video memory rather than 16MB.

  [There may be a third possibility.  It seems that a lot of SS7 boards
require a driver from the chipset manufacturer (VIA, SiS, ALi) in order to
properly support some AGP video cards.  This too may have gotten lost during
the re-install....]
