I sounds like you have essentially ruled out a cardiac
reason as well as acid reflux for the chest pain.  Gall
bladder problems and pancreatitis, two disease states
influenced by dietary changes could be the culprit.  I'd
recommend an ultrasound of the gall bladder as well as
amylase and lipase blood levels to check the pancreas.  Ask
your doctor to check these things out. Both of these things
commonly cause the referred pain between the shoulder
blades.
