I can answer this one.

We don't live alone.  We live with our parents, grandparents, and
great-grandparents, especially in traditional societies.  The
sicker they are, the more problems we have, and the less
attractive we are as mates.

This is not "adaptation to chronic diseases," of course, but it
illustrates the existence of selection pressure against diseases
that strike after procreation age.
