  Low-level formatting of IDE/EIDE drives is generally not
recommended by manufacturers.  A few have utilities for this posted
on their web pages; NONE endorse using the well-known ROM routine
accessible from DEBUG.

  This sounds, to me, like the drive is probably permanently damaged.
Probably cheaper to replace than to try to repair.
