Seconding Liza.  MD's who charge $90 for a 5 minute visit are also paying
for receptionist, nurses, hi-tech diagnostic equipment, etc.  A's fees are
analagous to a therapist with moderate overhead charging $300 -- not many
with the 'chutzpah' to do so!
