there aren't that many alternatives for mail.app in OS X. Those that are out there have varying degrees of accessibility (or the lack of it).

I do know this, try getting firefox to work on OS X Lion using a screen reader. 4 words that will always drive you crazy: NOT GOING TO HAPPEN.
