Beth and John,

The functional MRI or a SPECT scans sound like good ideas, but where
John lives, he may well have to go to Salt Lake (or maybe Seattle) to
get them.  The "joys" of living in MT or WY are also some of it's
negatives.  I don't know for sure what Boise, Idaho Falls or Billings
might have.  For those, I'd go to Denver.
