John,
Definitely try your church if you have one. Some Episcopal churches
do well at this, and LDS churches do, depending on the ward/stake and
leadership.
Service clubs are a great idea for an equipment purchase (usually a
one time expense rather than a recurring (monthly) expense. Has Mary
ever been to Shriner's in Salt Lake or Spokane (My Idaho geography
isn't great) If Mary's covered under any type of insurance, you could
try there, if you haven't already. Also check for local/state
foundations. There are a couple in Wyoming that work well with school
age kids. You could try to get it included in her IEP, although I
understand that Idaho's track record on those issues may not be
stellar.

I am working on getting some type of trust fund that could accept donations set up for any other equipment needs Mary may have in the future. Oh please pass the link along (only if your comfortable doing so). It still aggravates me that medicaid does not think a little child should enjoy the outdoors. I am looking into a local church to attend with my two boys, I can't get my wife to or Mary to attend though they both have major social phobia.

An unreasonable man (but my wife says that's redundant!)
The reasonable man adapts himself to the world; the unreasonable one
persists in trying to adapt the world to himself. Therefore, all
progress depends on the unreasonable man.
