One thing I thought was funny and an indication that something was messed up
with my settings was that I could hear a distinct hissy noise when I plugged
in my headphones. I don't quite have anything external to plug in to but
headphones was a good choice. And as you might have read already it was a
problem with my volume settings.

Kevin, first, plug your speakers into a radio or little player, to see if
they are working right.
2nd, try the other plugs on your sound card, to see if the jack is in the
wrong plug.
