It happened to me, but generally it only does if the ISP is the same as the 
outgling ISP.  That might provide a clue when she encounters that again.

It can happen, I know some one else that happened to and it has happened to
me. Some times especially if the SMTP server doesn't like an address, it
won't let it send.
