I played with the thing a bit and for no known reason it started charging 
just fine. I'm going to try it again in a day or two because I have no real 
idea what the problem is.  I would have said that there was a problem with 
the plug making a good connection but the light always comes on.  As for the 
sleeve, that doesn't seem to fit very well.  Going to try again when I'm not 
quite as annoyed with the silly thing.

Hi Rich, I had an Icom  quick charger, bc139A, and as I understand it,
1, the other/somewhat smaller  socket on the back of your Icom charger is
for use with the cp19R, the Icom t90's cig cord, the idea being that you
could A, use the quickcharger either using the included wallwart 117VAC, at
home or somewhere with an AC main, or B, while mobile, with the cp19R to
charge up your Li ion pack.  So if your wallwart doesn't seem to make the
charge light come on, and if you have one of those cords, and a ciglighter
output from a power supply or a car for that matter, see if the charger box
will work with it.
I don't believe you can have the box chargeing correctly with both power
input plugs in use at the same time...apparently, and sensibly, you either
use the wallwart or the cig cord...meaning that there's probably a circuit
in each of those plugs which will allow one to be engaged while the other is
not....and that may be where your problem is.  If you have the cig cord and
can make the box charge the bat while in the radio, or otherwise, with the
cord, then unplug it and try charging with the wallwart...you may have a
"switchover problem between the two plutgs.
2, I never noticed any kind of reset button at least on the outside of mine,
although there may be one somewhere inside the unit, if so, the charger's
paperwork, which I no longer have...1 little page if I remember correctly,
should say so, but so far in my reading of the manual, I've not found
reference to it.....although I'm certainly not through with it yett---and my
thanks to www.icanworkthisthing.com for a pretty jjaws friendly "from pdf
translation.

3, Finally,  In terms of the plastic sleeve, if I remember correctly, it
will only fill up the "big slot/well" on the charger base, two different
ways.  One allows the radio to slide down in it with the bat pack on it,
(although the manual tells you to turn the radio off while it's in there
chargeing,) with the terminals of the batpack snugged against the terminals
in the well.  The Other way the sleave fits, is, I suppose, designed to let
only the batpack itself fit down in there, again with the terminals
corresponding to the terminals in the well---{same terminals being used when
the radio's atatched to it, only using the sleeve to make a thinner insert
so that only the batpack itself fits snug in that configuration, where as
the batpack and radio fit snugly in the other.
