If all else fails, run the Windows installation procedure but select the
repair option instead of a new install. I have had to do this on more than
one occasion. Hate to do it without knowing why, but getting it fixed is
sometimes more important.

I have run the following from XP recovery consol with no progress:
chkdisk and chkdsk /r
fixboot
fixmbr
