  This usually means that the cable between the motherboard and the floppy
drive is plugged in reversed.  Some floppy drives aren't keyed to keep you
from doing that; nor are a few motherboards.

  Allegedly, you can fix this by unplugging the cable at the unkeyed end,
rotating the connector 180 degrees, and plugging it back in.  But every
floppy drive that I've ever misconnected this way (I've lost count) has been
permanently killed and has refused to operate when reconnected correctly....
